Jessica Castle, MD

Dr Castle is board-certified adult endocrinologist with 15 years of experience in clinical trials in the field of diabetes. She served as the Associate Director of the Harold Schnitzer Diabetes Health Center, where they provided care to 5,900 patients living with diabetes. She is also board-certified in internal medicine. Her clinical trial experience includes obtaining 11 investigational device exemptions from the FDA for automated insulin delivery and decision support studies. She has been on numerous data safety monitoring boards, including boards overseeing multiple large multi-center automated insulin delivery trials and for trials based both in the US and in Europe.

Dessi completed her PhD in Kinesiology & Health Science at York University under the supervision of leading diabetes researcher, Dr. Michael Riddell. She is currently working with Dr. David Maahs and the University of Stanford team to improve clinical outcomes in youth with type 1 diabetes.

Dessi Zaharieva's research focuses on effective strategies to reduce blood sugar fluctuations (i.e. hypoglycemia) during exercise and in recovery in individuals with type 1 diabetes.

Dessi has been living with type 1 diabetes for over 25 years and is also an award-winning high-level athlete within Brazilian Ju-Jitsu. She understands the challenges associated with exercise and blood glucose control and is very passionate about diabetes research, specifically the body’s response to different types of physical activity.

During the episode, Dessi runs through some clinical studies and interprets insights from them to provide practical and valuable advice on various types of exercise, insulin doses, and current glucose trends.

The speakers also explore the importance of continuous glucose monitoring in managing diabetes and the role played by patient-centered care. Finally, Dessi explores how technology advancements in health care have led to the development of tools that simplify guideline applications in practical real-time scenarios.

Multicenter Trial of a Tubeless, On-Body Automated Insulin Delivery (AID) System With Customizable Glycemic Targets in Pediatric and Adult Participants With Type 1 Diabetes (T1D)

Background

Automated Insulin Delivery (AID) systems have been shown to improve glycemic control and reduce hypoglycemia, but the burden of T1D is significant. This study focuses on researching the enhanced capabilities of a recent system.

Objective

The purpose of this study was to assess the safety and effectiveness of the first tubeless, on-body AID system.

Methods

This tubeless automated insulin delivery system was safe, and allowed participants to significantly improve A1C levels and Time in Range (TIR) with a very low occurrence of hypoglycemia.

† Recommendations from the International Consensus on Time in Range, 2019 recommend individualized glycemic targets for high risk and/or older adults with a focus on reducing the percentage of time spent less than 70 md/dL and preventing excessive hyperglycemia.
