With the widespread adoption of diabetes devices in the recent years there has been a change in the approach to assessing glycemic control metrics. In addition to A1C, many healthcare providers have turned to time in range as a new way to evaluate glycemic control.

Advances in diabetes care allow patients to monitor their glucose levels with great precision, and are leading experts to consider a new metric for glycemic control: time in tight range that has been defined as glucose levels between 70-140 mg/dL.

In this short ~17 min presentation Viral Shah, MD is going to provide a fresh outlook on time in range research. He will discuss:

Origins of time in range metric and its limitations

Overview of time in tight range

How TITR can help prevent chronic conditions

And where the future of the science lies

The information you see or hear at this event provides details about medical products which are not available in all countries, may have different indications, and/or have not received approval or market clearance by all governmental regulatory bodies throughout the world.

Despite advancements, many individuals with T1D struggle to maintain optimal glycemic control. Automated insulin delivery (AID) systems, particularly hybrid closed-loop (HCL) systems, aim to improve control and reduce burden by automating insulin delivery based on continuous glucose monitoring (CGM) data.

In this Research Spotlight we are highlighting a recent study:

Peacock, S., Frizelle, I. & Hussain, S. A Systematic Review of Commercial Hybrid Closed-Loop Automated Insulin Delivery Systems. Diabetes Ther 14, 839–855 (2023). https://doi.org/10.1007/ s13300-023-01394-5

The review followed the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) checklist. Studies focusing on single-hormone commercial hybrid AID systems, involving participants with T1D, and reporting endpoints related to glycemic control were included.

The following endpoints were the main focus of the review:

Participants with T1D

A1c levels

Diabetic ketoacidosis (DKA) events

% of Time in Range (TIR) / Change in TIR

Incidents of severe hypoglycemia

Commercial HCL systems effectively improve TIR and glycemic control in T1D, with minimal risk of severe hypoglycemia.

Studies showed that HCL systems generally improved time in range (TIR) and glycemic control without increasing severe hypoglycemia. Psychosocial outcomes, such as quality of life and treatment satisfaction, were generally positive.

Results (Glycemic Outcomes)

Commercial HCL AID Systems are effective and can improve TIR3

Improvements in glycemic control were shown without increasing the incidence of severe hypoglycemia

Glycemic outcomes supported by real-world studies and randomized control trials (psychosocial endpoints analyzed separately - showing generally positive outcomes, but more data needed)

Zaharieva, to discuss all things diabetes. Dessi completed her PhD in Kinesiology & Health Science at York University under the supervision of leading diabetes researcher, Dr. Michael Riddell. She is currently working with Dr. David Maahs and the University of Stanford team to improve clinical outcomes in youth with type 1 diabetes.

Dessi Zaharieva's research focuses on effective strategies to reduce blood sugar fluctuations (i.e. hypoglycemia) during exercise and in recovery in individuals with type 1 diabetes.

Dessi has been living with type 1 diabetes for over 25 years and is also an award-winning high-level athlete within Brazilian Ju-Jitsu. She understands the challenges associated with exercise and blood glucose control and is very passionate about diabetes research, specifically the body’s response to different types of physical activity.

During the episode, Dessi runs through some clinical studies and interprets insights from them to provide practical and valuable advice on various types of exercise, insulin doses, and current glucose trends.

The speakers also explore the importance of continuous glucose monitoring in managing diabetes and the role played by patient-centered care. Finally, Dessi explores how technology advancements in health care have led to the development of tools that simplify guideline applications in practical real-time scenarios.

Learn how professional health care specialists like Dessi harness the knowledge gained over decades of research to ensure that it translates into ready-to-use strategies for those with diabetes.
