With the widespread adoption of diabetes devices in the recent years there has been a change in the approach to assessing glycemic control metrics. In addition to A1C, many healthcare providers have turned to time in range as a new way to evaluate glycemic control.

Advances in diabetes care allow patients to monitor their glucose levels with great precision, and are leading experts to consider a new metric for glycemic control: time in tight range that has been defined as glucose levels between 70-140 mg/dL.

In another episode of Real Time Real Talk, host Cher Pastore invites Dr. Dessi P. Zaharieva, to discuss all things diabetes. Dessi completed her PhD in Kinesiology & Health Science at York University under the supervision of leading diabetes researcher, Dr. Michael Riddell. She is currently working with Dr. David Maahs and the University of Stanford team to improve clinical outcomes in youth with type 1 diabetes.

Dessi Zaharieva's research focuses on effective strategies to reduce blood sugar fluctuations (i.e. hypoglycemia) during exercise and in recovery in individuals with type 1 diabetes.

Dessi has been living with type 1 diabetes for over 25 years and is also an award-winning high-level athlete within Brazilian Ju-Jitsu. She understands the challenges associated with exercise and blood glucose control and is very passionate about diabetes research, specifically the body’s response to different types of physical activity.

During the episode, Dessi runs through some clinical studies and interprets insights from them to provide practical and valuable advice on various types of exercise, insulin doses, and current glucose trends.

The speakers also explore the importance of continuous glucose monitoring in managing diabetes and the role played by patient-centered care. Finally, Dessi explores how technology advancements in health care have led to the development of tools that simplify guideline applications in practical real-time scenarios.

Tadej Battelino, MD, PhD

Tadej Battelino obtained his MD at the Ljubljana Faculty of Medicine, where he also passed his board exam in pediatrics. He is currently a tenured professor of pediatrics at the Ljubljana Faculty of Medicine, and the head of Department of Pediatric endocrinology, diabetes and metabolism at the UMC - University Children’s Hospital Ljubljana, Slovenia. He serves as the Vice Dean for research at the Faculty of Medicine, University of Ljubljana, where he also serves as the Head of Chair of Pediatrics. He received numerous publicly funded grants in basic and clinical science.

Sufyan Hussain, MA, MB BChir, MRCP, PhD

Dr Sufyan Hussain is a consultant Diabetes and Endocrine Physician in one of the UK’s leading diabetes services at Guy’s and St Thomas Hospital and Honorary Senior Clinical Lecturer at King’s College London. He has a wide scope of practice both in the hospital and in community settings. His current research interests include hybrid-closed loops, understanding the support needs for using technologies effectively, diabetes distress and cell therapy in diabetes.

Dr Sufyan Hussain has over 31 years personal experience of living with type 1 diabetes and is passionate about improving care and advocacy for diabetes.

To date, there is limited clinical evidence about the benefits of CGM in pregnant patients. In this 25 min presentation, Carol Levy, MD, CDCES, will discuss currently available data on pregnant women with Type 1 diabetes, and share new data that came out of using the latest generation of real-time CGM.

In this 20 min presentation you will learn about Comparison of different treatment modalities for Type 1 diabetes Including Sensor-Augmented Insulin Regimens in 7 years of follow-up: the COMISAIR study.

COMISAIR study is currently the longest CGM clinical trial. Outcomes suggest solid evidence about benefits of continuous glucose monitoring. Below are the key takeaways from the study:

CGM provided significant and stable improvement of glycemic outcomes.
Treatment with CGM and MDI was more effective than PUMP therapy alone.
CGM and MDI can be an alternative to PUMPS and CGM but are inferior to AID.
The trial suggests a lower risk of diabetic retinopathy with CGM compared to SMBG.

Dr. Weissberg-Benchell will explore barriers to CGM use.

First, Stayce Beck, PhD, MPH, will talk about Human Factor studies and how they impact product development, specifically around safety and efficiency. She will discuss methods and principles of Human Factors testing for specific CGM systems, including recent Dexcom G7 Ease of Use Study.

Dr Weissberg-Benchell will then cover current views surrounding the psychosocial factors involving CGM use in PWD. The relentless nature of diabetes care, the inability to always understand or control glucose outcomes, and the impact of the daily burden on psychosocial functioning is a key driver in outcomes yet receives relatively little attention. While the metabolic benefits of CGM are well known, diabetes management is more than just metabolic outcomes.

Dr. Weissberg-Benchell is a pediatric psychologist with over 25 years of research experience and clinical work with children, adolescents and families with chronic illnesses and their families. Her academic appointment is as a Professor of Psychiatry and Behavioral Medicine at Northwestern University Feinberg School of Medicine. She is faculty at the Ann and Robert H. Lurie Children’s Hospital of Chicago and is part of the hospital’s consultation-liaison service.

She has published numerous research articles and offered workshops and lectures addressing issues such as improving self-management behaviors, family adaptation and coping, building adolescent resilience, the use of diabetes technology (insulin pumps, continuous glucose monitors and the artificial pancreas), and transitioning from pediatric to adult care. She is a co-author of two books, one that focuses on transitioning from pediatric to adult care, and another that focuses on working with adolescents with type 1 diabetes.
